You may not be able to use the router, if your moving from an ADSL service to Cable or ADSL2+ or the other way round your old router may not be compatible.
On the whole though if moving from ADSL to ADSL you can reuse the modem (I have several times), Cable to Cable will often need a new router as they use the MAC to authenticate against the network.
